does this bolt up to the two angle iron pieces that are welded to the frame? are these two angle iron weldments the "flanges" that are discussed in the assembly manual?

Yes but keep in mind the drive shaft is offset in the tunnel so attach the loop close to the drive shaft on the pass side, leave a 1/4" space between the loop and the shaft. You have plenty of room on the driver side.
